Urinary Proteome Monitoring for Transplant Injury

Abstract

We propose to achieve the objective of identifying novel urinary biomarkers for selected categories of renal transplant injury in two phases. We will first employ a discovery phase to comprehensively scan the urinary proteome by high-throughput proteomics, utilizing cross-sectional urine samples banked from renal transplant patients at Stanford University. In the second pre-clinical validation phase a large cohort of serially collected, banked urine samples at Stanford University from a multicenter prospective clinical trial, will be used to validate the biomarker proteins by high throughput LC-SRM-MS. The deliverable is a highly selected and validated set of urinary proteins and peptides that can be used to develop new assays or tests for serial, clinical monitoring patient urine samples in an out-patient setting, specific and sensitive for categories of graft injury relating to acute rejection (AR), chronic allograft nephropathy (CAN), stable transplants without significant injury (STA) and BK viremia/vyuria (BKV).
